Support staff

CDPD is home to a dedicated team of nurses who provide support to our clients. Our nurses are registered with AHPRA, have the required levels of professional indemnity insurance and are proactively engaged with professional development.

WHY IT WORKS

A smarter solution

Implementing an intentional patient management process allows your organisation to focus resources on the patient, instead of the process.

Better for practices

Working one-to-one with clinics, CDPD designs patient management strategies that result in enhanced clinic team engagement, long-term financial sustainability and a boost in chronic disease patient visits.

Better for patients

Around half of all Australians are dealing with a chronic disease. In order to best provide for this growing and complex need, CDPD engages patients in a collaborative plan to more effectively manage their chronic disease.

Better for staff

CDPD places nurses at the forefront of chronic disease management service delivery, allowing them to play a greater role in patient care. Being central to improved patient outcomes is especially gratifying for nurses, particularly when the service delivery is within the scope of their interests and skills.

Productive, profitable partnerships

CDPD was established with a vision to assist primary health care organisations to adopt a business model to preventative health care.

Hear from existing partners

Liz Dickson

General Manager, High Street Medical Clinic, VIC

A pathway to achieving better health outcomes

"Prior to using CDPD, the clinic struggled to maintain its base of chronic disease patients. As a result of in depth discussions, data reviews, interviews with nursing staff and GP meetings, we were able to collectively strip back the current processes."

Sonia Daniels

Practice and Business Support Coordinator, Rural Health West, WA

Sharing the benefits of nurse-led chronic disease management

"Donna has made an invaluable contribution to our practice and business support services. She has a very amicable style and explains things in a logical and organised manner. Donna is a delight to work with, and goes over and beyond to assist in any questions posed of her by the workshop and webinar participants. I highly recommend Donna as an educator, her professionalism is outstanding."

CDPD is a proven success across Australia in small, medium and large general practices
