The Primary Healthcare Chronic Disease Management program is designed to develop the knowledge and skills required for professionals working in general practice to address client needs, develop marketing strategies for chronic disease management and to develop and implement chronic disease self-management programs.
Address Customer Needs (BSBCUS402)
This unit of competency expects participants to apply organisation procedures and be aware of, and apply as appropriate, broader factors involving ethics, industry practice and relevant government policies and regulations
Develop Health Promotion Strategies for Chronic Disease Management (CDMDS805B)
This unit of competency expects participants to demonstrate the knowledge and skills required to assess, analyse, and develop health promotion strategies in a health care context, to individuals or groups in relation to chronic diseases and management.
This unit applies to clinicians where chronic disease is an important component of primary patient care and is applicable to the broader community. This would apply to individuals working in hospitals, primary care (general practice) and community health.
